Veterinarian Shortage Situation Nomination Form

State Animal Health Officials (SAHOs) must use the veterinary shortage situation nomination form for the nomination of veterinarian shortage situations to the Veterinary Medicine Loan Repayment Program (VMLRP). The veterinary shortage nomination form instructions document (found below) provides a series of screen shots for preparing, submitting and saving the fillable PDF VMLRP shortage nomination form. All shortage nomination form(s) must be submitted in a single email by emailing all completed PDF form(s) to VMLRP.applications@usda.gov by an authorized State or Insular Area Animal Health Official.
